Our connect-to-patient program is designed to allow health care providers to directly connect to the patient remotely. Connect-to-patient focuses on methods that utilize regional approaches, networks, health information technology, telehealth, or telemedicine to deliver services to individuals in rural areas and underserved environments.
Telehealth is defined as the use of electronic information and telecommunication technologies to support long-distance clinical health care, patient and professional health-related education, public health, and health administration. Technologies include video conferencing, the internet, store-and-forward imaging, streaming media, and terrestrial and wireless communications.
Health Risk Assessments
Annual wellness exams provide baseline data on an individual's health through the acquisition of vital statistics and complete blood analysis. The exams give a more specific look at a person's health status and can better identify immediate threats to their well-being or elevated risks for chronic diseases. A health risk assessment (HRA) is used to evaluate an individual's health risks and quality of life. The HRA serves as the first step in determining a person's risk factors for chronic disease or acute, potentially life-threatening conditions, but does not include blood draws for lab analysis. An HRA provides patients with valuable information regarding their health and provides tools necessary to move towards a healthier lifestyle.Rural Edge HRAs

of outcomes to now include an emphasis on comprehensive efforts, targeted outcomes, and collective responsibility.
It includes the personal, social, economic, and environmental factors that influence the distribution of health outcomes,
and the policies and interventions that affect those factors.
